• Description:

  HealthPAC is a program designed for people who need, but can't afford, health care and don't qualify for help from other programs. HealthPAC is only for Alameda County residents. Once enrolled in HealthPAC, individuals get coverage for most health care needs. Individuals may need to pay a co-payment for services depending on the service and their personal situation. HealthPAC is not health insurance. Individuals with health insurance are advised not to drop their insurance. HealthPAC eligibility is approved for up to one year, and must be renewed each year by updating enrollment information. Services covered may include:

• Eligibility: Must be a resident of Alameda County, and have limited income (income below 200% of the Federal Poverty Level), and have no health coverage, and health coverage does not cover the service you need.
• Requirements: Must show proof of identity, residency and income sources. If applicant is a U.S. citizen or National and has 5 years of Legal Residency, should also be able to show proof of citizenship.
