HEALTHPAC ENROLLMENT SITES, NATIVE AMERICAN HEALTH CENTER - OAKLAND

Description: HealthPAC is a program designed for people who need, but can not afford, health care and don't qualify for help from other programs. HealthPAC is only for Alameda County residents. Once enrolled in HealthPAC, individuals get coverage for most health care needs. Individuals may need to pay a co-payment for services depending on the service and their personal situation. HealthPAC is not health insurance. Individuals with health insurance are advised not to drop their insurance.

Eligibility: Must be resident of Alameda County with income below 200% of Federal Poverty Level, and no health coverage and/or health coverage that does not cover service needed.
Requirements: Must show proof of identity, residency and income sources. If applicant is a U.S. citizen or National and has 5 years of Legal Residency, should also be able to show proof of citizenship.
