EAST BAY WRAP, FRED FINCH YOUTH & FAMILY SERVICES

Description:
Offers a program that comprises three wraparound programs serving youth and their families. The three programs are: Alameda County Full-Service Partnership (FSP), Regional Center Wrap, and San Mateo FSP. Each program is an array of clinical and non-clinical services. Promote wellness, resilience and stability in the youth's placement. Services provided include:
- safety planning
- behavioral intervention
- family coaching
- individual coaching
- linkage to natural and community-based supports
- assessment of family/youth needs

Eligibility: Each of the 3 programs have different eligibility requirements.
For Alameda County FSP: youth must have full scope Medi-Cal and obtain approval through ACCESS. For Regional. Serves youth aged 8-18 who have had significant mental health or school difficulties. Youth are not connected to child welfare or probation systems. AC referrals are made through ACCESS.
Center Wrap: youth must be consumers of Regional Center of the East Bay and obtain approval through a Case Manager's request.
