ADULT RECOVERY OUTREACH AND CONNECTION (ADROC), TELECARE CORPORATION

Description: Provides short-term case management services to adults, ages 25 and older. Uses emergency rooms, inpatient settings, and crisis services to manage mental health and substance use needs. This program works with Alameda County residents who are not yet connected to county healthcare services. Staff provides linkages, supports, and resources to help participants remain in recovery-oriented settings.
