Description: Provides short-term case management services to individuals, ages 16 to 24. This program works with Alameda County residents who are not yet connected to county healthcare services. Staff provide linkages, supports, and resources. Help clients remain in the least-restrictive, most self-sufficient, and recovery-oriented settings possible.

Offers ongoing services during Public Safety Power Shutoffs (PSPS) and Disasters. Services are available if power is turned off in this facility.
