HEALTHPAC ENROLLMENT SITES, BAY AREA COMMUNITY HEALTH

Description: Offers a program for people who need, but can not afford health care and do not qualify for help from other programs. HealthPAC is only for Alameda County residents. Once enrolled in HealthPAC, individuals get coverage for most health care needs. All HealthPAC participants get a Participant ID Card, a Medical Home, and access to HealthPAC customer service. HealthPAC is not health insurance. Individuals with health insurance are advised not to drop their insurance.

Eligibility: Resident of Alameda County; income below 200% of Federal Poverty Level; and no health coverage and/or health coverage does not cover service needed. HealthPAC eligibility is approved for up to one year, and must be renewed each year by updating enrollment information.
Requirements: Must show proof of identity, residency and income sources. If applicant is a U.S. citizen or National and has 5 years of Legal Residency, should also be able to show proof of citizenship.
