ENHANCED CARE MANAGEMENT, ALAMEDA FAMILY SERVICES

Offers Enhanced Care Management (ECM) for community members living in or near the city of Alameda. ECM is a free benefit for Alameda County Medi-Cal recipients. Enhanced Care managers can support youth and their families up to 21 years old and adults 21 and older. Provides an assessment of needs and care coordination to identify, manage, and connect clients to helpful services and resources.
