Considers patient rights, quality of care, abuse, physical environment, and other related issues. Advocates for better care for all elders. Serves as a witness for nursing home residents signing their durable power of attorney for healthcare services.
Provides information and advice to those considering long-term care or trying to choose a facility. Provides advice and assistance with discharge planning for clients leaving long-term care.
Also provides training to nursing home personnel and develops resident councils.
Eligibility: Individual admitted to long-term care facility or considering long-term care, or family member of an individual admitted to or considering long-term care.
