Family/Household is not able to access other payment assistance for the same costs (no duplication of benefit)
Requirements: LIHEAP: Photo ID for ALL adults in the household Social Security Cards for ALL members of the household. (adults & children) PG&E bill (all of the pages) or an "Account Information Form" from PG&E or Utility bill from your Propane company. Proof of income for everyone in the home for the past 30 days. (Wages, TANF, SSA, SSI, Unemployment, Disability) Any and all income must be reported. Proof of Citizenship or Legal Residency of the person signing the application. (Birth Certificate, Passport, Residency Card)
