Point of Care Lab Services for A-1C
Podiatry and Foot Care
Retinal Screening
Wound Care
Patient Care
Insulin Pumps: Initiation, Management and Training
Diabetes Patient Education including New Onset Diabetes Classes
Assessments/Screenings
Family Medicine/Primary Care
Weight Loss Programs for Diabetics
Nutrition Therapy
Point of Care Lab Services: glucose, urine, hemoglobin
Continuous Glucose Monitors
Gestational Diabetes Education and Management
Also provides YMCA Diabetes Prevention Program where people at risk of developing Type 2 diabetes will learn how to eat healthier and increase physical activity.
Please note: General services are available during Public Safety Power Shutoffs (PSPS) and disasters. These services are still available if a power outage occurs in this facility.
Eligibility: Patients must be at least 18 years old or older regardless of the type of diabetes they may have, type 1, type 2, gestational, or any other special types of diabetes.
