Description:
Provides access to health care services for San Francisco adult residents who do not qualify for any health insurance plan. The program allows participants to choose from one of the primary care clinics located throughout the city for access to:
- emergency care
- urgent care
- ambulance services
- hospital care
- primary and preventive care
- pharmacy
- specialty care
- mental health services
- alcohol and drug treatment
- lab tests
- family planning
- durable medical equipment
- and more

Eligibility: A San Francisco resident Age 18 or over Uninsured Not eligible for public insurance programs such as Medi-Cal or Medicare Living in a household with an income at or below 500% of the Federal Poverty Level
Requirements: ID, Proof of residency, and proof of income
