- Eligibility: Must have at least one minor child in the home (up to age 19 if attending high school) Must be Native or Non-Native parent(s) with an eligible Native American child or be a Caretaker Relative with an eligible Native American child Must reside in San Luis Obispo, San Benito or Monterey Counties Must meet income eligibility requirements; countable income must be equal to or less than 200% of the Federal Poverty Level and families must have less than $5,000 in countable resources All adult applicants are required to complete a drug and alcohol screening at intake, including caretaker relatives
- Requirements: pplicants must submit the following documents with a completed application for consideration of services and assistance: .Tribal certification (Tribal ID, Certificate Degree of Indian Blood (CDIB), Letter of Tribal Descendancy .Residency verification (current utility bill and rental agreement/mortgage statement) .Identity verification for all members of the Family Assistance Unit: Valid photo identification cards (Adult only) .Birth certificates .Social Security cards .Children's Information Guardianship/custody documentation (if applicable) .Child Support information (paid, received or proof of application for child support required) .Updated immunization records for all children .School enrollment and attendance verification for all school-aged children . Proof of pregnancy with unborn child(ren)'s expected date of birth (if applicable) . Income verification for last 6 months (i.e., wages/ pay stubs, per capita payments, revenue sharing, worker's compensation, SSI, SSDI, SDI, SSP, VA, income tax refund, loans, gifts, spousal support, etc.) .Unemployment Insurance Benefits (EDD) or proof of application, if applicable, for the last 18 months Verification of potentially available income if applicable (i.e., Railroad benefits, VA, SSDI, etc.) .Aid verification (i.e., County Passport to Services, Tribal Commodities, housing subsidy, financial aid) .Resource and asset verification from last 30 days (i.e., bank statement(s), property deeds, life insurance, annuity statements, statements for stocks/bonds/money market accounts, etc.) .Vehicle information (registration, insurance information and payment verification
