ADULT DAY SERVICES, SELF-HELP FOR THE ELDERLY

Description:
Provides adult day services to seniors and adults with physical or mental impairments. Helps clients to continue living in their homes and communities. Also help those discharged from the hospital that need to be monitoring during short or long term rehabilitation.

Eligibility: Must be frail seniors (aged 65 and older) including those with Alzheimer's disease or other forms of dementia, or adults with disabilities (aged 18 and older).
Requirements: Written approval from doctor or physician. Proof of tuberculosis clearance in last 12 months.
