    Provides individuals with behavioral health issues with assistance living independently in the community. Services provided teach and enhance movement toward increased self-sufficiency. Offers services including:

    • Regular home visits, service providers educate and activate independent living skills. (Social services, counseling support, and transportation to medical appointments, coordinated with Napa County Mental Health.

    • Staff meets with clients individually. (Topics: symptom management, medication education, smoking cessation, nutrition and exercise, etc)

    • Supportive mental health rehabilitation services. (Assistance with individual needs for problem-solving, conflict resolution, coping skills, medication management, etc)

    • Crisis intervention is provided with 24-hour on-call accessibility.
