Description: Provide assistance to help refugees locate health services to evaluate and improve their health. Provides services which focus on health assessments to identify communicable diseases and to refer those in need of medical attention to health care providers. Working in collaboration with different agencies, assists refugees transition to life in America and become self-sufficient. Services include outreach, health screenings, health education, immunizations, tuberculosis follow-up and care, referrals to health and human services, interpretation, and health advocacy.
Eligibility: Contra Costa resident and refugee (refugee defined as a person who is unable to return to his or her own country because of persecution or a well-founded fear of persecution on account of race, religion, nationality, membership in a particular social group, or political opinion and has been officially given refugee status) Asylee, victim of trafficking and parolee.
