Description: Provides free surgical eye care for those individuals with no insurance and limited incomes. All clients must have a doctors referral. Areas covered include diagnostic testing, eye surgical care and eye medications. All services are performed in San Francisco at California Pacific Medical Center. The Foundation does not provide for routine eye exams, eye glasses, contact lenses or low vision aids.
Eligibility: Individual with no health insurance who is financially unable to pay for their eye surgery; no age restrictions; proof of income and medical physician form.
