CONTRA COSTA HIV/STI PROGRAM - CC HEALTH (SPECIALIZED INFORMATION AND REFERRAL), CONTRA COSTA HEALTH

Description: Provides a variety of services for HIV-infected individuals including medical case management, women's services, home health care; referrals to physicians experienced with HIV infection; help with transportation, food, housing/utility and other emergency assistance; and information and referral for other needed resources (such as substance abuse services, mental health therapy, adult day center care, etc.). Provides ADAP medications for financially eligible persons and PrEP navigation services (prevention) for HIV negative individuals.

Eligibility: Contra Costa County resident, HIV/STI diagnosis. Legal immigration status is not required.
Requirements: Identification is required, except during anonymous clinic.
