Description: Provides routine and preventive health care services including, family practice, internal medicine, pediatrics, immunizations, prenatal care, and women's health services, immunizations and laboratory (1st & 3rd Weds of the month). Care coordinator relays messages to doctors and assists patients with specialty referrals. LVN panel Coordinator assists patients with medication questions, refills, re-writes, etc. Patients can reach their doctor's coordinator by dialing backdoor numbers and leave a message.
