Description: Provides a day health program licensed by CA Dept. of Public Health for individuals 18 and over with disabilities or multiple chronic. Program offers a combination of medical, social and therapy services for fragile adults while giving their caregivers and families respite. The day is designed to increase and/or maintain current physical and mental functioning. Also provides individualized plans combining medical, social and therapy service for adults with mid to late stage Alzheimer's disease and dementia. The day is filled with appropriate activities for people with memory impairments, and may include live music, physical horticulture and art therapies, exercise and games.
