• Description: Provides permanent affordable housing solutions and vital support services to homeless and very low-income families and individuals in Contra Costa County. Partners with Mercy Housing to provide Permanent Supportive Housing for the most vulnerable at Garden Park Apartments (27 family units, Pleasant Hill). Also provides Permanent Supportive Housing in a multi-site program throughout the county (Richmond to Antioch) to 20 additional families and 48 individual adults. All Permanent Supportive Housing units are filled through the CC Coordinated Entry system. Homeless individuals can be placed on the Coordinated Entry list at the CARE Centers.
