Provides assessments to homeless individuals or families who are unable to get to a homeless CARE Center or individuals age 65+ and inputs their information into the County Homeless Coordinated Entry System. Answers dispatch line for Concord shelter, East County Interim Housing Program, and Concord Warming Center. Offers limited transportation to CORE clients only.
