• Description: Administers California State Disability Insurance (SDI), partial wage-replacement insurance plan for California workers. This office serves residents of Alameda and Contra Costa counties, regardless of place of employment. Applications available on-line or at local career centers. Applicant must have a doctor's certification of disability; an independent medical examination to determine the initial or continuing eligibility may be required. • Eligibility: California resident; must be unable to do regular work for at least 8 consecutive days, employed or actively looking for work at the time became disabled and have lost wages because of your, earned at least $300 from which SDI deductions were withheld during a previous period, must complete and mail a claim form within 49 days of the date you became disabled, must be under the care and treatment of a licensed physician/practitioner during the first 8 days of your disability and a doctor must complete the medical certification of the disability.
• Requirements: Licensed physician/practitioner must complete the medical certification of individual's disability. A licensed midwife, nurse-midwife, or nurse practitioner may complete the medical certification for disabilities related to normal pregnancy or childbirth.
