• Description: Provides Enhanced Care Management (ECM) services for Medi-Cal members with highly complex needs. ECM is a benefit that provides extra services to help clients get the care needed to stay healthy. Qualified individuals will have their own care team, including a Lead Care Manager who will talk to client, doctors, specialists, pharmacists, case managers, social services providers and others to make sure everyone works together to get client the care needed. Lead Care Managers can also help clients find and apply for other services in their communities.
