- Description: Provides Enhanced Care Management (ECM) services for Medi-Cal members with highly complex needs. ECM is a benefit that provides extra services to help clients get the care needed to stay healthy. Enhanced Care Management services help to coordinate clients care. A Lead Care Manager will be assigned who will work with the patient, doctors, specialists, mental health providers, and other care staff to make sure everyone is working together. Lead Care Managers can also help clients find and apply for other services in their communities which include, medically tailored meals and housing supports, help patients understand and manage medications, help with scheduling appointments, find and apply for community services and work with patients and their family to improve their health.
