    The Housing and Disability Advocacy Program (HDAP) helps homeless, disabled individuals apply for disability benefit programs, while also providing housing supports. Services include:

    • Housing supports, including a housing navigator and temporary and permanent housing
    • Disability advocacy
    • Homeless prevention services, such as home repairs, utility payments, and durable goods purchases
    • Wraparound services, such as food, clothing, and other essential need items
    • Case management and outreach services

    The Home Safe Program assists individuals involved with Adult Protective Services (APS) who are experiencing, or at imminent risk of experiencing, homelessness due to elder or dependent adult abuse, neglect, self-neglect, or financial exploitation. Services include:

    • Temporary supports, including a housing navigator and temporary and permanent housing
    • Homeless prevention services, such as home repairs, utility payments, and durable goods purchases
    • Wraparound services, such as food, clothing, and other essential need items.
    • Intensive case management, including service coordination, problem-solving, education, and more

    The Bringing Families Home (BFH) offers financial assistance and housing-related wraparound supportive services to families involved with the child welfare system who are experiencing, or at risk of, experiencing homelessness.

    • Financial assistance including rental assistance, utility payments, security deposits, and moving costs
    • Housing supports including a housing navigator, case manager and temporary or permanent housing
    • Other services that may assist in securing housing for families including, legal services and credit repair

    The Transitional Age Youth Housing Program (TAY) Program encompasses three programs to prevent homelessness and secure and maintain housing for young adults aged 18 to 24 years, inclusive, with priority given to those currently or formerly in the foster care or probation systems.
