    This program supports Indigenous communities accessing health and navigation services in Fresno County. We focus on assisting our community in accessing COVID-19 vaccines, COVID-19 testing, blood pressure and glucose checks, medication refills, referrals within the community, and conducting case management. Additionally, we conduct monthly community meetings and host semi-monthly Rural Mobile Health Clinics in different rural towns in Fresno County.
