Description: Provides assistance to eligible aged, blind and disabled individuals who are unable to remain safely in their own homes without this assistance. Offers an alternative to out of home care, such as skilled nursing or board and care.Works with the Connecting Point Public Authority to provide a list of contracted IHSS care providers including community care providers, friends, neighbors, and relatives. Services provided could include: household chores, personal care services, transportation and accompaniment to medical appointments, protective supervision, paramedical services as ordered by a physician, help with mobility inside and outside the home, and other services per State guidelines.

Eligibility: Must be Medi-Cal eligible, permanently disabled, or 65 years old or older. Must live in their own home, which means the place in which the individual chooses to reside. This does not include an acute hospital, skilled nursing, board and care or intermediate care.
