Description: The Senior Outreach Nurse program provides health services to county residents 60 years and older. Offers comprehensive health and safety assessments, health screenings, fall prevention education, medication education/management resources, and continued in-home follow-up. In-home assessments review a client's medical, social, and environmental needs and offer health education consistent with client's needs. Will discuss referrals that are appropriate for clients, and help link them to other community resources and services. Provides follow-up visits and telephone calls as needed.Offers outreach such as community based blood pressure clinics, flu shots in conjunction with Public Health, and participate in local health/wellness programs.
