To ask for Medical Transportation please call Anthem Blue Cross Transportation Reservations at 877-931-4755 at least five business days (Monday-Friday) before your appointment or call as soon as you can when you have an urgent appointment. There is no advance requirement for dialysis, chemotherapy, radiation therapy, urgent care, wound care or discharges.
