From these results, we construct a lifestyle care plan that can be easily followed and updated as the client continues to age and circumstances evolve. She made the transition because it seemed to her that families needed more help coping with a relative's dementia or Alzheimer's disease than patients did. Kristina consults one-on-one with clients about their family's unique needs, and provides a wealth of information and referrals to care communities that best fit their situation.Silver Pathways is contracted with Seniors First and pays Seniors First a portion of their income to help fund and support the nonprofit.
