• Description: The Union Labor Health Foundation, a supporting organization of the Humboldt Area Foundation, provides funds for this program. It was created in 2001 to meet dental-related needs of youth in Humboldt County where emergency funds are required. Use the "Dental Angel Fund" Request Form to make your request. You will be notified within two weeks whether your request was approved. Service providers and advocates may not make requests for their own reimbursement and individuals may not apply on their own behalf. The dental fund committee meets at least once per month. Advocates are encouraged to obtain an application by fax and submit an application by fax, thereby allowing more time for consideration. Please call 442-2417 prior to submitting a request.
