• Description: Helping frail older adults to stay as healthy and independent as possible requires a partnership between these individuals and their care team. At Redwood Coast PACE each member's health, quality of life and well-being are our top priority. We provide older adults with a broad range ofhealth and supportive care services that are convenient, simple to use, reliable and well coordinated. Redwood Coast PACE - a Program of All-inclusive Care for the Elderly - is a comprehensive health plan for eligible older adults who wish to remain living in their home and community. We provide medical, social, nutritional, rehabilitation, and support services for frail adults who: Are age 55 and older Need a nursing home level of care (the state of California determines/certifies each applicant's need for a nursing home level of care as part of the enrollment process) Can safely live in the community with no harm to themselves or others, and Reside in the Redwood Coast PACE service area - Eureka, McKinleyville, Arcata, Bayside, Blue Lake, Carlotta, Ferndale, Fields Landing, Fortuna, Hydesville, Korbel, Loleta, Rio Dell, Samoa, Scotia, or Trinidad
