Description: St. Joseph Home Care coordinates every aspect of a patient's care, including communication with the hospital and the insurance company. This includes helping you with insurance forms and paperwork. We develop and coordinate a complete home care program, beginning the moment we're contacted by a patient's physician, hospital discharge planner, family or the patient themselves. We provide an age range from pediatric care to chronically and critically ill geriatric patients. We also provide meal preparation, companionship, respite workers to relieve and instruct primary caregivers at home, social workers to help patients cope with emotional trauma that occurs due to illness, speech therapy for stroke victims, and day-to-day observation of changes in the general health of our patients--just about every service that can safely be provided in the home. AIDS program, wound care, cardiovascular care services, case management, counseling, dressing changes, foley catheter insertion and care, home health aide (personal care, housekeeping, meals), home ventilator patient care, injections, I.V. therapy, occupational therapy, pastoral care, pediatric care, peer counseling, physical therapy, psychiatric nursing, respiratory care, skilled nurses, social services, speech therapy, support group referral, trans-neural stimulation for pain control, tracheostomy care.
