Description: Using a combination of a fully equipped mobile van and ODCHC's eight fixed clinic sites, homeless individuals receive comprehensive care to include:General medical care for all agesFamily PlanningMental /Behavioral HealthSubstance abuse treatmentCase management servicesSkilled nursing servicesLaboratory servicesHealth educationReferrals to community resourcesOur mobile unit is a custom designed 38 foot motor home with two exam rooms, a restroom, lab, pharmacy, a wheel chair lift and is supplied with the same equipment as a family doctor's office. The fully staffed mobile unit visits each community site on a regular schedule. We offer medical services to patients of all ages, including patients who are homeless, in transitional housing, or patients who have difficulty scheduling appointments. We also provide medical services to migrant and seasonal farm workers and their families.We offer walk-in appointments in addition to our scheduled appointments at all locations.After Hours CareIf you have an urgent medical problem call (707)443-4666 when the clinic is closed and your call will be transferred to a provider. Medical providers will advise if an Emergency Room visit is necessary and give you medical advice.
