• Description: Provides one-on-one support to families with children (pregnancy through five years of age). Offers to pair families with a nurse or family support specialist to help support parents and families. Services include weekly home visits that helps with the areas described below.   Families are eligible to enroll from pregnancy up to two months postpartum, with special extended eligibility for those who are low income, receiving Medi-Cal, or those whose babies are at risk of adverse childhood experience
