Description: Provides medical care coordination to assist families who are low-income. Assists with medical appointment scheduling, transportation, and access to diagnostic and treatment services. Also provides access to providers who give health check-ups to infants, children, and youth in Mendocino including:
- Developmental screening: identifies children who should receive more intensive assessment or diagnosis for potential developmental delays.
- Immunizations
- Dental Screening
- Vision Screening
- Hearing Screening
