Eligibility: Children between the ages of 0-5 years.
Requirements: Child's Birth Certificate, Income Verification, Proof of Residence, Immunization Record and proof of TB clearance, Copy of Child's Physical, Proof of Child's Dental Exam, Health Insurance Card, Proof of Family Size, Adult TB clearance.
