• Eligibility: For Patient: 1) If over 18 years of age - one year's continuous residency in the communities served by the Lions Eye Foundation (LEF) and must be a legal resident of the United States. Under 18 years of age - one year's continuous residency in the communities served by the Lions Eye Foundation (LEF). 2) Income requirements: (based on net income) Single / Married Couple $22,800 Single Parent w/ 1 child $25,200 Single Parent w/ 2 children $27,600 Single Parent w/ 3 children $30,000 Add $2,400 for each additional child 3) Patient has no insurance / coverage for eye surgical care.
