Description: Enhanced Care Management is a benefit for Medi-Cal members. It comes from a new Medi-Cal program called California Advancing and Innovating Medi-Cal also called CalAIM. This benefit helps members with complex needs coordinate their health care. As an ECM provider, we will be working to connect clients with an ECM team from one of our contracted providers. This team includes a lead case manager. The lead case manager will talk with doctors, mental health providers, specialists, pharmacists, case managers, social services providers, and others. They will make sure everyone works together to get the client the care one needs. A lead care manager can also help one find and apply for other services in the community. Finding a doctor if needed scheduling appointments for health related services. Managing medications help to make sure you have the medications you need. Setting up a ride to go to the doctor or pharmacy. Finding and applying for community based services that you need. These services can include housing or medically nutritious food.
Eligibility: Clients will need to be enrolled in Medi-Cal with the Central California Alliance for Health, eligible clients are referred from CCAH contracted providers Dorothy's Place, Central Coast Center for Independent Living, Sun Street Centers, Monterey County, Midpen Housing, Salud Para La Gente, Health Projects Center, Titanium Health, Salinas Valley Health, La Casa Adult Day Center, Mom's Meals, Modify Health
