Eligibility:
Identification Document (ex. state-issued driver's license/ID or passport)
Proof of home/work address (ex. utility bill, lease agreement addressed to applicant)
Proof of storm hardship (letter that housing was affected by the 2022-2023 storms or letter from employer indicating loss of employment or reduced work hours
If the documents are not available, a self attestation form will be required
