This agency offers Community Supports for Medi-Cal members. These supports address social factors that can contribute to health challenges, such as lack of stable housing. By connecting members with essential resources, this agency helps improve overall health and quality of life for those facing difficult circumstances. Those resource include in sequential order:
Eligibility: Individuals experiencing *homelessness (sheltered or unsheltered)
Pregnant/postpartum women.
Children up to the age of 21 y/o (who are homeless)
*Homelessness can be considered the following:
. An individual or family who lacks adequate nighttime residence;
. An individual or family with a primary residence that is a public or private place not
designed for or ordinarily used for habitation;
. An individual or family living in a shelter;
. An individual exiting an institution into homelessness;
. An individual or family who will imminently lose housing in the next 30 days;
. Unaccompanied youth, homeless families, and children and youth defined as homeless
under other federal statutes; or
. Individuals fleeing domestic violence.
