- Description: Coordinates services for persons of all ages with developmental disabilities and their families. Disabilities may include cerebral palsy, epilepsy, autism, and other similar conditions. Provides the following support, services, and resources to eligible individuals and their families:Case managementDevelopmental evaluationsHearing, medical, speech, vision, genetic, nursing, and nutritional evaluations.Diagnosis and coordination of resourcesEducation, health, benefits, rehabilitation, and recreation.Prevention program for infants under the age of three who are at risk for developmental disabilities.Developmental services for children ages birth to five. Serves those who have a significant delay in two or more developmental areas.Infant development programs, parent education, family training, speech therapy.Psychological servicesAssistive technology servicesAdult day programSupported employment for adultsIndependent living skills for adultsSupported Living Services for adultsLicensed Residential Facility placement (with limited ability to place youth under age 18)In-home and out-of-home respite care servicesFunding for health services (not covered by other funding sources)Genetic counseling when appropriateLegal and financial servicesMay include court-ordered evaluations, limited conservatorship, representative payee services.
