- Description: Advocates for and coordinates health care needs of foster children and at-risk adults. Provides services at home, school, in the community, or at the Child Welfare Services office. Services include: Nursing assessments of infants, children, and adults at risk Nursing interventions Case management Referrals to medical, dental, mental health, and other support services Creating and updating a Health or Education Passport (health record for children) Health education and information to parents, social workers, and substitute care providers. These include foster parents, group homes, and relative placements.
