- Description: Provides youth transition program for people with disabilities ages 14 to 30. Offers the following services: Tools, and support to transition from school-based services to independent living Information and referrals Guidance with center and community services.One may also have the opportunity to qualify for a paid Peer Mentor Internship. Services help one with: Education planning - connects to local education programs and resources Exploring a variety of job fields and finding out if one has what it takes to do the job Being more independent - teaches skills like budgeting, healthy eating, and personal organizing Improving social skills - explores how to make new friends in the community and at one's job.
