Requirements: -For appointments:-Photo ID/DL for each adult in household-Social Security cards for you, spouse and dependents-Forms W-2 & 1099 -Proof of insurance coverage & 1095A-Last year's tax return-Total paid daycare provided and daycare tax id-All deductions, including charitable contributions and credits-Bank account and routing number for direct deposit
