  • Description: Serves Partnership HealthPlan members who are homeless or at risk of homelessness. Specifically, helps those coming out of rehabilitation programs, hospitals, mental health facilities, or incarceration. Integrates Enhanced Care Management (ECM) and Community Supports (CS) under CalAIM. Provides services in-person at their Redding office or over the phone. They also travel in-person to those in Shasta, Humboldt, Butte, Lassen, Siskiyou, Tehama, and Trinity County. Services include: Short-term housing (up to 6 months) - for those exiting incarceration or substance use disorder (SUD) rehabilitation programs.Recuperative Care (up to 6 months) - for those discharged from hospitals, mental health facilities or SUD treatment programs.Enhanced Care Management (ECM) (up to 1 year and can be extended) - for those age 12 and older. Helps connect and schedule mental health, physical health and SUD-related care. Housing Transition Navigation Services (HTNS) (up to 6 months, extendable) - for those age 17 and older. Helps with obtaining and maintaining long term housing.Housing Deposits (one time use) - for those age 17 and older, requires enrollment in an ECM program (not necessarily through this agency).Intensive Drug and Alcohol Program (10 week course) available to any member receiving services through New Life Discovery Project.Assistance with obtaining identification and benefits: Help securing ID cards, Social Security cards, birth certificates, and applying for General Assistance, EBT, SSI, and other benefits.Referrals and coordination: Links to mental health and substance use treatment providers, employment services, and other community resources.Care Planning: Development of individualized care plans based on each client's needs and goals.Offers a variety of in-person groups and activities for clients including:Fellowship groupMeditation groupWorkout groupRecovery groupParenting group (for clients who are or have worked with CPS)Yoga groupActivities such as computer lab, guitar lessons, housing class, pizza/games and more.
