  • Description: Provides enhanced case management services for youth and adults. This is a Medi-Cal benefit available to eligible members with complex needs. Services include: Services for Adults: Homelessness support and resources Assistance for those at-risk of avoidable hospital utilization Transition support for nursing facility residents moving back into the community Long-term care (LTC) eligibility services for those at-risk of institutionalizationThis may include home-based services, community-based programs, and residential care. Support for adults transitioning from correctional facilities to community living. Services for Youth and Families: Assistance for youth, and families experiencing homelessness Support for those enrolled in CCS (California Children's Services) programs Services for youth involved in the welfare system (foster care). These services help promote stability, health, and community inclusion for those in need.
