Services by a Care Manager may include:Case managementCoordinating health care with doctors, dentists, optometrists, and specialistsHelp with understanding prescriptionsHelp arranging medical transportation.Serves the following populations:Unhoused adults wanting to transition into permanent housingAdults transitioning from a correctional facility to community livingYouth enrolled in CCS (California Children's Services) programsYouth involved in the welfare system (foster care).
