Description: Telecare Los Angeles Hopes and Opportunities Program (LA HOP) operates under contract with the Los Angeles County Department of Mental Health and offers mental health services to adults with serious mental illnesses who are at risk for hospitalization or institutionalization, are homeless or at risk of homelessness, face criminal justice involvement, have not been successful with traditional mental health programs, and those transitioning from institutional settings.
