Description: Case management services are available to patients accessing HIV medical care at St. John’s. The trained case managers assist patients in the areas of housing, transportation, public benefits, medication adherence, disclosure, and risk reduction counseling; as well as provide referrals to dental services, behavioral health services, substance abuse counseling and treatment, nutrition therapy, and legal assistance.
Eligibility: St. John's accepts Medi-Cal, Medicare and many other insurance plans. There are benefits counselors at all of the health centers to help patients determine their eligibility for insurance programs.
