Description: The case managers become the primary contact for their patients, coordinating care and gaining access to services and scheduling appointments. Case managers help connect patients to a variety of resources including: medical care, dental care, behavioral health services, housing, childcare, food assistance, financial support, etc. St. John’s currently offers case management services to eligible prenatal, pediatric and HIV care patients in the Los Angeles area.
Eligibility: St. John's accepts Medi-Cal, Medicare and many other insurance plans. There are benefits counselors at all of the health centers to help patients determine their eligibility for insurance programs.
